If you have out-of-network benefits through your PPO plan, I can provide a superbill for you to submit directly to your insurance for reimbursement. If you plan to use out-of-network benefits, I recommend you contact your insurance provider to  ask about reimbursement.

To an extent, I offer sliding scale on a limited basis.

My fees are comparable to others in my area with similar training and experience.
